Predictors of Severity in Scrub Typhus. J Assoc Physicians India. 2019 Apr;67(4):35-38 Authors: Sharma R, Mahajan SK, Singh B, Raina R, Kanga A Abstract Aims: To study predictors of severity in patients of scrub typhus admitted in a tertiary care hospital. Material and Methods: Total 92 patients of scrub typhus were included in the study. The diagnosis was established by presence of IgM antibodies by Indirect Immunofluorescence Assay (IFA) test which is currently the reference standard for the diagnosis of scrub typhus. The clinical and laboratory profile, course in hospital, and outcome were documented. Factors associated with severe disease were analyzed. Observations: Fever (100%), cough (37%), headache (33%), vomiting (31%), altered sensorium (23%), diarrhea (18%), abdominal pain (16%), myalgia (14%), and seizures (3%) were common clinical features. An eschar was present in 23% of patients. Common laboratory findings included elevated transaminases (61%), thrombocytopenia (39%), and leukocytosis (30%). Severe sepsis was present in 33% patients. Septic shock was present in 4% patients. Presence of one or more organ failure was seen in 34% of patients. The overall case-fatality rate was 4%. ConclusionOverall, the number of complaints was significantly reduced. Only in the case of persisting adenomyosis patients suffered from ongoing postoperative complaints, as in reproductive aged women keeping the uterus was highly required. Although this pilot study about systematic posterior peritonectomy showed improved recurrence rate and fertility rate, the main question remains: does this surgical technique appropriate better results and outcomes in the future. This question has to be addressed in a prospective randomized study.

ConclusionMindfulness was previously shown to be a promising intervention for chronic pain treatment. Our study demonstrates that higher pre-operative dispositional mindfulness is associated with more favorable post-operative pain outcomes, including lower pain scores, but not lower opioid consumption. This relationship provides an opportunity to target the modifiable personality characteristic of mindfulness, to improve post-operative pain for patients planning gynecologic surgery.

We present a series of four adult sickle-cell patients who developed PRES. The association of hypertension, renal failure, and immunosuppressants in sickle cell patients may play a role in the development of PRES in this patient population. It is unclear if SC patients have a higher frequency of PRES predisposing factors than the general population or if SC is an independent risk factor for PRES. Despite this limitation, the presence of acute neurological status changes in adult SC patients should trigger suspicion for PRES as management and outcome may be altered.
