High Value Care for Older Adults with Complex Care Needs: Leveraging Nurses as Innovators

Both nationally and globally, the number of older adults (people 65 years of age or older) is growing faster than any other age group. There were 49.2 million older adults in the United States in 2016 and by 2060, that number is projected to almost double to 98 million. (Administration on Aging) Most older adults in the United States have at least one chronic health condition, and close to half of this group are living with four or more chronic conditions. (Buttorff et al., 2017) The presence of multiple chronic conditions (MCCs) in the older adult population introduces many challenges.
