Late-onset Vibrio vulnificus septicemia without cirrhosis.

Late-onset Vibrio vulnificus septicemia without cirrhosis. Proc (Bayl Univ Med Cent). 2019 Apr;32(2):286-288 Authors: Lee MT, Dinh AQ, Nguyen S, Krucke G, Tran TT Abstract Recent surveillance from the Centers for Disease Control and Prevention indicates rising annual incidence rates of Vibrio vulnificus infection. Unfortunately, this infection is often excluded from the differential diagnosis in lesser known at-risk populations. Transmission occurs via wound exposure or ingestion, with V. vulnificus foodborne illness having the highest mortality rate of all Vibrio species. Fatality rates of V. vulnificus rival those of Ebola and bubonic plague, so timely treatment is imperative. Current literature favors surgical debridement with a third-generation cephalosporin plus intravenous doxycycline or fluoroquinolone. Cephalosporin monotherapy is discouraged due to rising resistance. This case features V. vulnificus septicemia with prolonged incubation time in a noncirrhotic patient. PMID: 31191157 [PubMed]

Conclusion: Most infants with BA have liver fibrosis at diagnosis and it progresses rapidly. The presence of cirrhosis is correlated with the age at surgery, which suggests the importance of this clinical variable in the evolution of fibrosis.RESUMEN Objetivo: Analizar la evoluci ón de los hallazgos histológicos de pacientes con atresia de vías biliares (AVB), resaltando la progresión de la fibrosis y comparando la biopsia hepática diagnóstica (BHD) con la biopsia hepática quirúrgica (BHQ) mediante la hepatoportoenterostomía de Kasai. ra Grieco The biological clock controls at the molecular level several aspects of mammalian physiology, by regulating daily oscillations of crucial biological processes such as nutrient metabolism in the liver. Disruption of the circadian clock circuitry has recently been identified as an independent risk factor for cancer and classified as a potential group 2A carcinogen to humans. Hepatocellular carcinoma (HCC) is the prevailing histological type of primary liver cancer, one of the most important causes of cancer-related death worldwide. HCC onset and progression is related to B and C viral hepatitis, alcoholic an...

Cystic fibrosis (CF), the most common autosomal recessive disease in Caucasians, is caused by mutations of the CF transmembrane regulator protein (CFTR) gene. Loss of function mutations of CFTR-mediated chloride and bicarbonate transport in the apical membrane of epithelial cells lead to impaired mucociliary clearance and accumulation of mucus in various organs, resulting in chronic airway disease, pancreatic insufficiency, malabsorption, biliary cirrhosis, and infertility [1 –2]. Individuals with CF are classically at risk for malnutrition, which is related to inadequate intake, increased energy expenditure, and malabsorption [3].

AbstractPBC is a chronic progressive autoimmune disorder involving the destruction of intrahepatic small bile ducts, cholestasis, fibrosis, and ultimately cirrhosis if left untreated. It is largely driven by the autoimmune response, but bile acids and the intestinal microbiota are implicated in disease progression as well. The only drugs licensed for PBC are UDCA and OCA. UDCA as a first-line and OCA as a second-line therapy are safe and effective, but the lack of response in a significant portion of patients and inadequate control of symptoms such as fatigue and pruritus remain as concerns. Liver transplantation is an end...

Authors: Stasi C, Sadalla S, Carradori E, Monti M, Petraccia L, Madia F, Gragnani L, Zignego AL Abstract BACKGROUND. New direct-acting antiviral drugs can eradicate hepatitis C virus (HCV) infection in over 90% of patients and can even reduce the risk of complications in advanced fibrosis/cirrhosis. The aims of this study were to evaluate 1) changes in fibrosis during and after antiviral treatment and 2) incidence of hepatocarcinoma and mortality in various fibrosis stages.METHODS. This is a longitudinal monocentric prospective study. Blood and instrumental examinations were evaluated at baseline, at the end of the...
