Anti-Neutrophil Cytoplasmic Antibodies Positivity and Anti-Leukotrienes in Eosinophilic Granulomatosis with Polyangiitis: A Retrospective Monocentric Study on 134 Italian Patients

Conclusion: Despite the limitations of this retrospective study, the association between LTRAs and ANCA antibodies elucidates the mechanism by which innate immunity is directly involved in tolerance breakdown and autoantibodies production. Validation of our results with targeted studies could clarify the differences between ANCA-positive and ANCA-negative patients with important consequences on the use of some drug classes in the treatment of EGPA and asthmatic subjects.Int Arch Allergy Immunol

Rituximab is a chimeric monoclonal anti-CD20 antibody with wide use in cancer (lymphoma) and connective tissue diseases (rheumatoid arthritis, and vasculitis)1. Hypersensitivity reactions to rituximab can range from type I IgE-mediated reaction (urticaria, shortness of breath, hypotension, or anaphylaxis), to cytokine-mediated reaction (fevers, chills, rigors), or a mix of both, thus limiting the use of rituximab as first line therapy for many cases. Desensitization is a new therapeutic approach which can be performed with rituximab to offset these reactions.

ConclusionsOmalizumab improved asthma control in some patients with EGPA with uncontrolled asthma by reducing asthma exacerbations and oral steroid requirement. However, more data are needed before recommending widespread use of omalizumab in patients with EGPA.
