47-Year-Old Man With Pancytopenia and Fever

A 43-year-old man from northern Michigan with anti –cyclic citrullinated peptide–positive rheumatoid arthritis (RA) diagnosed 23 years previously presented to his local primary care physician with asymptomatic jaundice. His RA symptoms included polyarticular joint pain in the hands, ankles, knees, and hips. These symptoms were well controlled wi th methotrexate and hydroxychloroquine. He had never had jaundice before and had no diagnosis of hepatic dysfunction.
