Pulse Pressure Variation Is Affected by PETCO2 in Children with Intracardiac Left-to-Right Shunt

We read with great interest the study by Deflandre et al.,1 which demonstrated the influence of sternotomy on dynamic indicators of cardiac preload. Dynamic parameters, including pulse pressure variation (PPV), have been incorporated into clinical practice in children with heart disease. Renner et al.2 found that PPV was a superior parameter to predict fluid responsiveness both before and after repair of congenital heart disease. However, there are some interferences that should be recognized when applying these parameters, such as arrhythmias, low tidal volume, and sternotomy.
