Hypertension Management in the Very Old

In 2017, the American College of Cardiology (ACC)/American Heart Association (AHA) guidelines for treatment of hypertension were published.1 Since then, much controversy continues to exist around whether and to what extent they should and should not be applied to older persons, including nursing home residents, with multiple comorbidities, frailty, and relatively short life expectancy. This has led to persistence of the philosophy among many physicians that the dangers of intensive blood pressure treatment in older persons outweigh the benefits in most, if not all, persons age 75  years and older.
