Consistent achievement of life-supporting cardiac xenograft function in a preclinical model is the benchmark established by an International Society for Heart and Lung Transplantation expert advisory panel in 2000 as a precondition for clinical translation.1 This long-elusive goal appears to be close at hand based on a recent report by L ängin and colleagues,1 published late last year in Nature.2 The Munich-based team reported that 4 of 5 consecutive baboon recipients of orthotopically implanted genetically modified pig hearts survived beyond 90 days.

In the early 19th century, the German scientist and poet Johann Wolfgang von Goethe published his celebrated drama entitled Faust, A Tragedy. In this drama, Faust, frustrated by his inability to gain infinite knowledge, strikes a deal with the demon, Mephistopheles. In doing so, while Faust is pursuing his ambitions, tragedy befalls his beloved ones, which leaves him unfulfilled. Such a Faustian bargain is an example of intended good that results in less ideal outcomes.

DISCUSSION: Passenger lymphocyte syndrome after mismatched organ transplantation is not rare, but the syndrome associated with non-ABO antibodies occurs in a much smaller subset of these cases. The mixtures of organ donor, recipient, and other transfused red blood cells profoundly limit the usefulness of serological testing. Lung transplant recipients (LTRs) have a high risk of developing Clostridium difficile infection (CDI), which can affect both graft and patient survival. Modalities to prevent post-transplant CDI are needed. We hypothesized that primary prophylaxis with oral vancomycin would be an effective preventive strategy. We assessed the association between oral vancomycin primary prophylaxis and CDI incidence by comparing the incidence rate of CDI in LTRs administered prophylaxis with that of LTRs given no prophylaxis.

In a previous issue of the journal, Apostolo et al.1 report on the impact of left ventricular support device (LVAD) speed change on several physiologic parameters, including maximal and sub-maximal exercise performance, sleep disordered breathing, and alveoli gas exchange. The patients were supported by the Jarvik device (Jarvik Heart, Inc., New York, NY), a unique chronic LVAD that permits patients to modify speed to meet work demands. The investigators used a variety of methodologies from several protocols to assess the effect that change in speed had on physiologic parameters.

In this study, we explored whether umbilical mesenchymal stem cell (UMSC) derived exosomes could prevent aging-induced cardiac dysfunction and determined whether the potential mechanism was mediated by the exosome/lncRNA MALAT1/NF-κB/TNF-α pathway. We discovered that human umbilical cord mesenchymal stem cell- (UMSC-) derived exosomes prevent aging-induced cardiac dysfunction. Silencer RNA against lncRNA MALAT1 blocked the beneficial effects of exosomes.
