The role of DEFB126 variation in male infertility and assisted reproductive technique outcome

ConclusionsThis sequence variation of DEFB126 may impair male reproductive function and can be related to male infertility. Consequently, the fertility rate is significantly less in del/del males than wt/wt males. Interestingly, males with the del/del genotype have normal spermogeram however, their sperm is functionally impaired, which can affect IUI treatment outcome.

Azoospermia, or the absence of spermatozoa in the semen, is present in up to 10% of men presenting with infertility. The majority of these men will have nonobstructive azoospermia (NOA), or impaired production of sperm. When present, testicular sperm can be used with in  vitro fertilization and intracytoplasmic sperm injection to achieve fertility. One of the most effective treatment options to obtain testicular sperm is microsurgical testicular sperm extraction (micro-TESE). Reported rates of successful sperm retrieval with micro-TESE range from 40% to 60%.

CONCLUSIONS: Our study demonstrated that approximately one in five patients in clinical practice showed discordance between AFCs and AMH levels. In view of the AFC being better than AMH for predicting POR, the AFC should be the preferred indicator for predicting ovarian response to subsequently develop an optimal individualized COH protocol.
