UNITED NATIONS, New York –“I have met many women who have suffered tremendously during labour,” said Dr. Kenza Aden, a general practitioner in Djibouti. This is because most of her patients – like about 78 per cent of girls and women aged 15-49 in Djibouti – have undergonefemale genital mutilation (FGM).  “I’ve even seen women bleeding to death,” she continued. “And that’s why I’m scared of getting married and having children.”

A 50-year-old female with a history of infected left total hip arthroplasty underwent left total hip prosthesis reimplantation. The patient developed massive bleeding intraoperatively, and an emergent angiogram was requested. Pelvic arteriography demonstrated multiple areas of active hemorrhage arising from branches of the left gluteal artery and profunda femoris treated with Gelfoam (Pfizer, Washington, DC) and coil embolization. Despite embolization, the patient remained hemodynamically unstable, requiring a massive transfusion protocol.

CONCLUSION: The combination of temporary balloon occlusion and uterine artery embolization does not seem to promote better hemorrhage control than each procedure performed individually does.

ConclusionIn patients with RPOC without previa, 61 and 39% did and did not require hemostatic interventions, respectively. In the latter, a wait-and-see strategy resulted in the resolution of RPOC. Patients with larger RPOC (> 4-cm fragment length) and hypervascularity were significantly more likely to require hemostatic intervention.

ConclusionChitosan covered gauze is an excellent option for treating PPH, it appeared to be at least equivalent to the balloon tamponade, in our experience particularly suitable for atony or placenta bed bleeding after spontaneous delivery or during cesarean sections, in cases of lower uterine segment atony, placenta previa bed bleeding, and/or coagulopathy.
