This study compares the safety, pain level, and time involved in these techniques. Methods: We randomized 100 patients receiving ultrasound-guided unilateral thoracentesis in an academic medical center from December 2015 through September 2017 to either vacuum or manual drainage. Without using pleural manometry, the effusion was drained completely or until the development of refractory symptoms. Measurements included self-reported pain before and during the procedure (from 0 to 10), time for completion of drainage, and volume removed. Primary outcomes were rates of all-cause complications and of early termination of the...

We read with interest the article published by Arnold et  al1 examining the effect of timing of physical therapy (PT) for acute low back pain on health services utilization (HSU). Reducing overuse of imaging, opioids, injections, and surgery for low back pain is an international priority.2 That early access to PT could replace or reduce many of these ser vices is not an unreasonable proposition, and one that is worthy of rigorous scientific investigation. However, we feel that by emphasizing observational studies over randomized controlled trials in their synthesis of available literature, the authors have, in this ca...

We would like to thank the authors of the Letter to the Editor for their interest and insight into our review, “The Impact of Timing of Physical Therapy for Acute Low Back Pain on Health Services Utilization.”1 We agree that to make more definitive causal claims about the outcomes from the timing of physical therapy, randomized control trials (RCTs) are needed. The final version of the abstract will refl ect a modification that removes causal claims. We believe that future research in this area should focus on using rigorous methodological designs that include randomization and observational studies that use la...

Background: We aimed to evaluate the functional outcomes, incidence of bone tunnel laceration, and bone tunnel morphology of transosseous arthroscopic rotator cuff repair (ARCR) with the Arthro-TunnelerTM. Methods: Between May 2012 and April 2013, patients with full-thickness tears who underwent transosseous ARCR with the Arthro-TunnelerTM and could be followed for a minimum of 2 yr were included in this retrospective study. Functional outcomes, cuff repair integrity, intraoperative bone tunnel laceration, and postoperative bone tunnel morphology were analyzed. Results: We retrospectively evaluated 42 consec...

ConclusionSelective bowel resection algorithm provides a systematic approach in the surgical management of patients with bowel endometriosis. Adherence to the surgical plan according to the pre-operative imaging and criteria outlined in the algorithm can be accomplished in the majority of patients. The surgical team, however, should be aware that upstaging or downstaging may be required depending on the intra-operative findings. When feasible, the team should opt for a less extensive procedure to avoid complications associated with more radical surgery.
