ConclusionsWe suggest that the mechanisms underlying sitagliptin-mediated inhibition of OAT vasculopathy probably occur through a direct increase in GLP-1 activity. In addition to the GLP-1-dependent pathway, sitagliptin may regulate SDF-1 α levels and EPC function to reduce OAT-induced vascular injury. This study may provide new prevention and treatment strategies for DPP-4 inhibitors in chronic rejection-induced vasculopathy.

Cytomegalovirus (CMV) infection is the most common clinically significant viral infection among CMV seropositive (R+) recipients of allogeneic hematopoietic cell transplant (HCT) [1]. CMV is associated with substantial morbidity and mortality, particularly in recipients of T-cell depleted (TCD) allografts and human leukocyte antigen (HLA)-mismatched or unrelated donor allografts [2-4]. The pre-emptive therapy (PET) approach is broadly used for CMV management [1]. In this approach, HCT recipients are routinely monitored for CMV post HCT and anti-viral therapy is initiated upon detection of CMV infection.

Conclusion: Considering the high prevalence of UTI and antibiotic resistance rates in kidney transplant patients in our study, there is an urgent need for developing hospital based local antibiogram for appropriate management of UTI. Fosfomycin as an empirical therapy might be a useful choice for adequate coverage of potential pathogens at our centre.
