We appreciate Dr. Grzybowski's interest and comments on our article “Central retinal artery occlusion – a new, provisional treatment approach” which was not a “preferred practice pattern” but a rational, provisional guide for management of a blinding condition in the absence of good evidence-based therapy.3 As we discuss in the article, treatment of comple te CRAO of more than about 15 minutes of duration is most likely without value; the generally accepted 240-minute window of opportunity is based on very flawed experimental evidence.
