Current state of dialysis access management in Korea. J Vasc Access. 2019 May;20(1_suppl):15-19 Authors: Kim YS, Kim Y, Shin SJ, Lee HS, Kim SG, Cho S, Na KR, Kim JK, Kim SJ, Kim YO, Jin DC Abstract The prevalence rate and the incidence rate of hemodialysis and functioning kidney transplant recipients have continuously increased; on the contrary, those of peritoneal dialysis have continuously decreased since 2006. Dialysis patients have been getting older and have been maintained on dialysis longer. Diabetic nephropathy was the leading cause of end stage renal disease. The type of hemodialysis vascular access has been stable during the last 5 years (arteriovenous fistulas 76%, arteriovenous grafts 16%, central venous catheters 8% at 2016). Peritoneal dialysis catheter was mostly inserted surgically (67%), and swan neck straight tip peritoneal dialysis catheter was the most commonly used (48%). Vascular access was managed by radiologists and surgeons, and the management was fragmented among them in the past. However, since the nephrologists became interested in and knowledgeable about the vascular access, they began to play roles in vascular access management. Conclusion: Our studies for the first time demonstrated that the acetate produced from gut microbiota mediated the dysregulation of cholesterol homeostasis through the activation of GPR43, thereby contributing to the tubulointerstitial injury of DN, suggesting that gut microbiota reprogramming might be a new strategy for DN prevention and therapy.

A 60-YEAR-OLD, 154 cm, 88 kg woman with essential hypertension, poorly controlled type II diabetes mellitus, and a previous kidney transplantation for end-stage renal disease resulting from diabetic nephropathy presented to a community hospital for evaluation of dyspnea and exercise intolerance. The physical examination was notable for an irregularly irregular rhythm and a grade II of VI holosystolic murmur best heard at the left lower sternal border. CONCLUSIONS In diabetic ESRD patients selected for renal transplantation, elevated pre-transplant HbA1c levels, defined as HbA1c>7.7%, are not predictive of post-transplant medication compliance. CONCLUSION: DM and kidney function affected energy and nutrient intakes. Subjects in the Late-CKD group consumed less energy than those in the Early-CKD group. Non-DM subjects seemed to restrict protein intake starting from the Early-CKD stage than subjects with DM. Subjects in this study had low energy and high sodium intakes compared with recommended levels. Protein intake was lower in advanced CKD patients, but their intake level was still higher than the recommendation.
