Cardiac CT Assessment of Right and Left Ventricular and Valvular Function

AbstractPurpose of ReviewThe purpose of this review is to summarize the recent technical and scientific advancements in cardiac computed tomography (CT) assessment of ventricular function and valvular heart disease.Recent FindingsWith improvement in scanner technology and protocols, cardiac CT can be used to assess cardiac function and chamber volumes with excellent accuracy and interobserver reproducibility. CT is a reliable adjunct modality to assess most valvular heart diseases when other modalities are not adequate or provide discrepant data. CT is now the standard of care in planning transcatheter valve interventions.SummaryCardiac CT can be reliably used to assess right and left ventricular function albeit with radiation exposure. CT is also helpful as a tiebreaker in complex valvular heart disease when other modalities are discrepant or limited. Further improvement in temporal resolution and reduction of radiation dose will increase utilization of cardiac CT with the goal of comprehensive cardiac assessment.

ConclusionsDue to the relatively high prevalence of abnormal stress MPI, tailored protocols with a stress-first MPI as well as the use of 2-day protocols and advanced imaging technologies including CZT SPECT, novel image reconstruction software, and PET MPI could substantially reduce radiation dose in complex CHD.

(Society of Nuclear Medicine and Molecular Imaging) Research presented at the Society of Nuclear Medicine and Molecular Imaging's 2019 Annual Meeting draws a strong link between severe obstructive sleep apnea (OSA) and impaired coronary flow reserve, which is an early sign of the heart disease atherosclerosis. Using 13N-ammonia positron emission tomography/computed tomography (PET/CT), researchers were able to noninvasively evaluate coronary microvascular function in OSA patients and use their findings to predict cardiovascular disease risk.

Conclusions: The present study shows the significance of estimation of myocardial blood flow and myocardial flow reserve in diabetic patients asymptomatic for CAD. Based on our study, rest and stress MPI using 13N-NH3 cardiac PET serves to be a valuable modality in early detection of coronary microvascular dysfunction in asymptomatic diabetic patients. This modality could be of great help in understanding both the coronary blood flow abnormalities as well as the detection of coronary artery disease in diabetic patients asymptomatic for coronary artery disease.
