Repeated Cerebrospinal Fluid Removal Procedure in Older Patients With Idiopathic Normal Pressure Hydrocephalus Ineligible for Surgical Treatment?

I read with great interest the article by Rossi et  al in which they illustrate the positive responses obtained with repeated cerebrospinal fluid (CSF) tap procedures in patients with normal pressure hydrocephalus (NPH) who were ineligible for surgical intervention with shunting.1 In the Introduction section, the authors have commented on the rapid loss of the benefits gained with the surgical procedure in NPH patients because of the parallel progression of comorbid conditions.1,2 They have also stated that there are a substantial number of patients who are ineligible for shunting surgery because of their advanced age and accompanying comorbi dities.
