Chronic Stress and Ovarian Function in Female Childhood Cancer Survivors. Oncol Nurs Forum. 2019 May 01;46(3):E75-E85 Authors: Hardy TM, Garnier-Villarreal M, Ohlendorf JM, McCarthy DO Abstract OBJECTIVES: To explore the relationships among perceived stress, biomarkers of hypothalamic-pituitary-adrenal (HPA) activity, gonadotropin levels, and anti-Müllerian hormone (AMH) in female childhood cancer survivors (CCSs). SAMPLE & SETTING: 24 female CCSs from the Royal Hospital for Sick Children in Edinburgh, Scotland, were included in the study. METHODS & VARIABLES: Perceived stress was measured using the Perceived Stress Scale. HPA activity was measured using salivary cortisol and hair cortisol. Ovarian function was measured using serum gonadotropin levels and serum AMH levels. Latent growth curve modeling was used to determine diurnal cortisol slope and intercept. Bayesian structural equation modeling was used to explore the relationship among perceived stress, biomarkers of HPA activity, and ovarian function. RESULTS: The authors found an inverse association between perceived stress and ovarian function and a positive association between biomarkers of HPA activity and ovarian function. IMPLICATIONS FOR NURSING: Further research is needed to understand factors contributing to risk for post-treatment reproductive dysfunction in female CCSs. PMID: 31007257 [PubMed - in process]

We present a case in which a patient in her late 30s was diagnosed with a Sertoli-Leydig cell tumour in the background of a strong family history of multinodular goiter and Sertoli-Leydig cell tumour with a germline mutation in DICER1. Case Presentation: A 38-year-old woman with history of multinodular goiter was found to have stage iiic ovarian Sertoli-Leydig cell cancer after presenting with abdominal pain. The decision of whether or not to use MHT after cancer is complex. It depends not only on indication (menopausal symptoms), but also on age and hormone dependence of the tumour. For example, squamous cell tumors of the cervix and vulva and hematological malignancies are not hormone dependent. However, breast tumours may express estrogen and progesterone receptors. Radiotherapy and chemotherapy can lead to premature ovarian failure, especially after cancer in childhood, leading to associated health consequences.

In this study, we reviewed major human studies on the health risks of radiation exposure and showed that sex-related factors may potentially influence the long-term response to radiation exposure. Available data suggest that long-term radiosensitivity in women is higher than that in men who receive a comparable dose of radiation. Introduction: Ovarian masses represent the most frequent tumors of the female genital tract in children and adolescents, about a half of them are neoplastic. A quarter of all tumors is malignant, additionally 2 –5% of those accounts for granulosa cell tumor (GCT).
