The Case | Microangiopathic hemolytic anemia in a young female

A 23-year-old female presented with headache, a blood pressure of 220/130 mm  Hg, and a creatinine level of 698 μmol/l, elevated from 87 μmol/l 6 months ago. She had microangiopathic hemolytic anemia, a platelet count of 75 × 109/l, a hemoglobin level of 95 g/l, and a lactate dehydrogenase level of 768 IU/l. Schistocytes were present on the peripheral blood smear, and the haptoglobin level was low (0.8 g/l). There was no history of diarrhea, viral illnesses, or new medications. Her urine dipstick demonstrated microscopic hematuria (degree of positivity [+]) and proteinuria (++; not formally quantified).
