The human duodenal mucosa harbours all components for a local renin angiotensin system

Conclusion: Immunostaining and pharmacological data demonstrate the presence of a local RAS in the human duodenal mucosa with capacity to influence epithelial ion transport by way of particulary the AT2R.

We read with great interest the article by Marya et  al1 reporting a randomized controlled trial (RCT) on early deployment of a video capsule for detecting the bleeding source. One current guideline suggests selecting the first endoscopy based on stool color, for both upper and lower GI bleeding.2 However, physicians cannot always detect the definit ive bleeding source at the first endoscopy in clinical practice.

The objective of the study was to evaluate the clinical safety of endoscopic browlift alone or in combination with blepharoplasty as a treatment for the aging face. This is a retrospective comparative study from 2007 to 2016 at a single tertiary care center. A consecutive sample of patients undergoing surgery for aging of the upper face was included in the study. Surgeries deemed to be inclusive were endoscopic browlift, upper blepharoplasty, and lower blepharoplasty. In total, 46 patients undergoing endoscopic browlift with concurrent blepharoplasty were included. Patient demographics, past medical history, follow-up, and...

In this study we used gold-standard measurements to demonstrate that patients with a posteriorly-located tongue (natural sleep endoscopy) exhibit a preferential improvement in collapsibility (lowered critical closing pressure, Pcrit) with oral appliances. We also show that patients with both posteriorly-located tongue and less severe collapsibility (predicted responder phenotype) exhibit greater improvements in OSA severity (reduction in event frequency by 83%, in contrast to 48% in predicted non-responders). The present study suggests that the structure and severity of pharyngeal obstruction determine the phenotype of sle...

Mackenzie, Matthew Rogers, Michelle Gallagher, Timothy RockallAbstractAcute gastrointestinal bleeding is a common medical emergency, accounting for approximately 85,000 admissions in the United Kingdom per annum. It is associated with significant morbidity and mortality. GI haemorrhage is commonly categorized according to source of blood loss; either upper GI (above the ligament of Treitz) or lower GI (below the ligament of Treitz). Rapid assessment, resuscitation and correction of coagulopathy should be undertaken to stabilize th...

Endosc Int Open 2019; 07: E1175-E1183 DOI: 10.1055/a-0914-2749 Background and study aims Our academic lab has developed a novel, low-cost, disposable endoscope for assessment of the esophagus and stomach without need for large equipment or complex electronics. Usability and intuitiveness of the platform are unknown. Methods The novel endoscope (NE) consists of a high-definition camera, LED module, and three bellows. Compressed air actuates the bellows, producing camera/LED articulation. Insufflation and lens cleaning ports are present. Video can be displayed on any monitor. Total material costs less than $...

Conclusion: Considering the relatively proper specificity and positive predictive value of BUN/Cr ratio, in cases that bleeding source cannot be determined using other non-invasive methods, values higher than 35 can predict upper GI bleeding with high probability. However, due to the low sensitivity, values less than 35 are not diagnostic. PMID: 31432040 [PubMed]
