P wave dispersion and silent atrial fibrillation in cryptogenic stroke: the pathogenic role of inflammation.

CONCLUSIONS: In cryptogenic stroke, high PWD is partly due to systemic inflammation that increases AF risk possibly via atrial electric remodeling. These findings could also suggest inflammation as a possible therapeutic target in order to prevent atrial electrical alterations and finally AF occurrence in CS. A 68-year old man with a history of hypertension, diabetes, coronary artery disease and an obliterating lower limbs arteritis, presented for a stroke. Electrocardiogram did not show atrial fibrillation nor ischemic electrocardiographic signs. White blood cell count (10800/m3) was high but eosinophil count (234/mm3) was normal. Atherosclerosis origin of the stroke was suspected.
