Authors: de Boysson H, Liozon E, Ly KH, Dumont A, Delmas C, Aouba A Abstract OBJECTIVES: To estimate the frequency of different clinical patterns in giant-cell arteritis (GCA) at onset. METHODS: All GCA patients consecutively followed-up in two referral centers for GCA with a biopsy-proven diagnosis and/or large-vessel vasculitis (LVV) demonstrated on imaging were analysed. RESULTS: We analysed the initial clinical presentation of 693 patients with a median age of 75 [48-94] years and including 486 (70%) women. We identified four different clinical patterns: isolated cranial GCA (in 80%), symptomatic LVV wi...

As the physician prepared to leave the exam room, the patient's wife spoke up:"Doctor, my husband won't tell you this, but he is suffering," the woman said, her voice cracking. Dr. Timothy Quan, a rheumatologist in central Connecticut, looked at the 69-year-old man he'd been caring for over the past several months. The man gave a brisk nod. It was true. The past few weeks had been a nightmare of pain.Six months earlier, the patient woke up with a sore, swollen right hand. He figured he must have injured it a few days before when he cleared out a pile of wood in his backyard. He mentioned it to his pri...

ConclusionsLVV is not uncommon, clinical features and elevated CRP levels should raise suspicion of LVV associated with PR. 18F-FDG PET/CT is useful in identifying LVV associated with PR.ResumenObjetivoLa polimialgia reumática (PMR) puede asociarse a vasculitis de grandes vasos (VGV).

PTX3 is a prototypic soluble pattern recognition receptor, expressed at sites of inflammation and involved in regulation of the tissue homeostasis. PTX3 systemic levels increase in many (but not all) immune-mediated inflammatory conditions. Research on PTX3 as a biomarker has so far focused on single diseases. Here, we performed a multi-group comparative study with the aim of identifying clinical and pathophysiological phenotypes associated with PTX3 release. PTX3 concentration was measured by ELISA in the plasma of 366 subjects, including 96 patients with giant cell arteritis (GCA), 42 with Takayasu’s arteritis (TA)...

Publication date: Available online 23 May 2019Source: Best Practice &Research Clinical RheumatologyAuthor(s): Lindsay Lally, Robert SpieraAbstractPolymyalgia rheumatica (PMR) and giant cell arteritis (GCA) represent a family of systemic inflammatory diseases occurring in adults aged 50 years and above. Clinical presentation of PMR/GCA can be variable, making diagnosis at times challenging. There has been an increased appreciation of the role of various large-vessel imaging modalities to help confirm a diagnosis of GCA. Systemic corticosteroids (CS) remain the mainstay of treatment for both PMR and GCA, yet both relapse...

DiscussionIt is important to consider the possibility of an underlying malignant process in the differential diagnosis since its early identification can be determinant for prognosis.

647Objectives: In advanced melanoma, combined immune checkpoint inhibitors (ICIs) against cytotoxic T-lymphocyte-associated-antigen-4 (CTLA-4, Ipilimumab) and programed cell death-1 (PD-1, Nivolumab) have higher efficacy and more immune-related adverse events (irAEs) compared to single agent PD-1 therapy. We evaluated the role of FDG-PET/CT in response assessment and irAEs detection following first-line combined ICIs therapy in advanced melanoma. Methods: Between 2015 and 2018, 121 patients received nivolumab (1 mg/kg of body weight) plus ipilimumab (3 mg/kg) every 3 weeks for four cycles, followed by nivolumab (3 mg/kg) e...

In this study, we retrospectively analyzed data of a group FUO/IUO patients that suspected of PMR, aiming to analyze the imaging features of PMR in FDG PET/CT and evaluate the clinical value of FDG PET/CT in differential diagnosis of PMR. Materials and Methods: From January 2014 to June 2018, among the patients who underwent PET/CT examination because of FUO/IUO, 26 patients were suspected of PMR due to shoulder joint pain and stiffness, increased serum inflammatory factors and age over 50 years old. Clinical and imaging data of these 26 patients were retrospectively analyzed. Final diagnoses were made by board-certified r...

In this study, we investigate predictive factors which correspond to subsequent disease control of PMR and RS3PE syndrome. Twenty four patients, which contained 18 PMR and 6 RS3PE syndrome, were treated with initial dosages of 10-20 mg per day oral prednisolone, and the dosage of prednisolone was then tapered. Significantly higher initial CRP was observed in patients with poor disease control than in those with good disease control afterwards. The number of patients with negative CRP after 4 weeks was significantly more in patients with good disease control after 1 year than in those with poor disease control. Patients wer...
