Sarcoidosis and idiopathic pulmonary fibrosis: The same tale or a tale of two diseases in one

We read with interest the letter submitted by Bianchi et al. regarding patients similar to those we recently described with combined sarcoidosis and idiopathic pulmonary fibrosis (CSIPF) [1]. We appreciate the interest in CSIPF and the presentation of additional patient cases. The question remains whether CSIPF represents a distinct phenotype, a coincidence or whether UIP is the common final pathway of fibrotic lung diseases such as sarcoidosis as the UIP pattern is not unique to IPF. Reports such as this demonstrate the need for further study of such patients.
