We appreciate Dr. Grzybowski ’s interest and comments on our article “Central retinal artery occlusion – a new, provisional treatment approach” which was not a "preferred practice pattern" but a rational, provisional guide for management of a blinding condition in the absence of good evidence based therapy.3 As we discu ss in the article, treatment of complete CRAO of more than about 15 minutes duration is most likely without value; the generally accepted 240 minute window of opportunity is based on very flawed experimental evidence.
