Double Helical Inferior Vena Cava Duplication

A 41-year-old woman with a history of venous thromboembolism was referred for inferior vena cava (IVC) filter placement before discontinuation of anticoagulation for upcoming surgery. IVC duplication was recognized, and a retrievable filter was placed within each infrarenal IVC moiety. A year later, the patient returned for filter removal. Simultaneous bilateral iliac venography demonstrated an unusual IVC variant (Fig 1a –c). The bilateral common iliac veins crossed each other, rather than uniting, at the expected location of the confluence and spiraled up as duplicated IVCs before joining just below the renal veins.
