Malignant and premalignant colorectal lesions in the 40s: The new generation to be screened?

We read with great interest the recent article by Cha et  al1 about endoscopic surveillance in young adults with sporadic colorectal adenomas. This issue, and anticipation of the age of screening for colorectal cancer (CRC), have been of special interest in our center, motivated by the fact that we are facing, in recent years, an increasing number of CRC cases at earlier ages, with apparently worse characteristics and outcomes. Although recent guidelines do not specifically address this issue, according to these more aggressive clinical and histologic characteristics by the time of diagnosis, we consider that screening for precursor lesions should be done in a way that allows effective prevention.
