Outcome after surgery in supratentorial and infratentorial solitary brain metastasis

ConclusionsThis study identified factors that play a role in the outcome after surgery in patients with ISM and SSM on MRI. Postoperative complications seemed to be higher in ISM and CM was more often seen in ISM, but the worse prognosis in patients with ISM compared with SSM could not be confirmed.

Authors: Okimoto T, Tsubata Y, Hotta T, Hamaguchi M, Nakao M, Hamaguchi SI, Hamada A, Isobe T Abstract The central nervous system is a common site of relapse in patients receiving crizotinib, which is presumed to be associated with the low concentration of crizotinib in the cerebrospinal fluid (CSF). Our patient received surgical treatment for anaplastic lymphoma kinase-positive stage IIA lung adenocarcinoma. His cancer recurred with brain metastases and carcinomatous meningitis. We started whole-brain radiation therapy (WBRT) and subsequently administered crizotinib. The concentration of crizotinib on day 15 in th...

Rationale: Serratia marcescens (S. marcescens) is an opportunistic pathogen of the Enterobacteriaceae family. Although S. marcescens is known to cause sepsis, meningitis, endocarditis, urinary system and ocular infections, skin infections are sporadic. Squamous cell carcinoma (SCC) is the most aggressive skin cancer type that is often located in the head and neck region, and rarely in the scalp tissue. Patient concerns: An 89-years-old male patient was diagnosed with SCC three years ago. The frontal region of the skull showed an ulcerated tumor, irregular borders, and exophytic growth pattern. The destruction of the f...

Authors: Ashinuma H, Shingyoji M, Hasegawa Y, Yokoi S, Yoshida Y Abstract The mechanisms underlying anaplastic lymphoma kinase (ALK) resistance have not been well investigated in clinical practice. We herein report the case of a lung cancer patient with carcinomatous meningitis who had an ALK I1171T resistance mutation revealed by direct DNA sequencing of the cerebrospinal fluid after treatment with cytotoxic chemotherapy, crizotinib, and alectinib. I1171T is considered to be sensitive to ceritinib. Although ceritinib was not effective initially, we chose ceritinib again after whole-brain radiotherapy and ventricul...

AbstractTo present an unrecognized vascular complication of bacillus Calmette-Gu érin (BCG) therapy administered for superficial bladder carcinoma. We also review the potential mimickers for primary angiitis of the central nervous system (PACNS) as well as complications of intravesical BCG therapy. An 89-year-old Caucasian man with a history of relapsing high-grade bladder carc inoma treated with intravesical BCG presented with recurring episodes of right upper limb paresthesia with clumsiness and dysarthria. Magnetic resonance imaging of the head revealed multiple predominantly left-sided frontotemporal micronodula...

We present the case of a woman with a progressive gait disturbance that started immediately after having been treated with docetaxel for breast cancer resembling the same clinical characteristics as seen in patients affected by normal pressure hydrocephalus. The symptoms had been observed for about 2 years as having been caused only by polyneuropathy, given the high incidence of this side effect and the accompanying numbness of distal extremities. However, brain MRI evidenced a marked enlargement of the ventricular system. Brain metastases as well as carcinomatous meningitis were ruled out. After having placed a ventriculo...

Conclusions This report illustrates an unusual presentation of reversible MTX leuknoencephalopathy due to Ommaya reservoir malfunction. Early clinical review and imaging post instillation of chemotherapy through the Ommaya reservoir maybe beneficial to detect early complications.

CONCLUSIONS:SNUC have a high rate of CNS invasion at presentation as well as at recurrence. Involvement of the CNS is associated with poor outcomes and represents a cohort in need of improved therapy.

Conclusions: There are few reports of carcinomatous meningitis from linitis plastica. This may be due to a high early mortality rate, before malignant cells can reach the CNS. We suggest that patients with a history of gastric adenocarcinoma presenting with CNS complaints warrant an initial work-up with brain imaging, where findings of cranial nerve enhancement should increase suspicion for CM, and LP, focused on cytology. Unfortunately, CM in the setting of gastric adenocarcinoma has an overall poor prognosis.Disclosure: Dr. Narula has nothing to disclose. Dr. Masangkay has nothing to disclose.

I wrote this in the form of a letter a few days ago. Doing so has been a huge relief to me. I know that it's not the same for everyone but expressing myself in this way has been both calming and freeing.I have sent it already to family and to friends with whom I have been speaking. It helps a lot because we can start the discussion from here and I don't need to explain from the beginning each time. Or not talk about this stuff at all, in the knowledge that they know. It works for me.You should also know that I learned all this on March 4th (and suspected before because of the symptoms I was having). The news was worse...

The objective of radiation therapy is to improve quality of life. Association with systemic therapy improves overall survival. Administration of sequential intrathecal chemotherapy may also improve overall survival, but induces more toxicity. The use of new radiotherapy techniques and development of radiosensitizing molecules in patients with good performance status could improve survival in this frequent complication of cancer.
