Dichotomizing the spectrum of ductal shunt places long-term outcomes research at risk

We commend Collins et  al on a comprehensive analysis of extensive follow-up data designed to evaluate the long-term implications of a neonatal diagnosis of patent ductus arteriosus (PDA).1 We agree that the literature is lacking thoughtful, well-designed studies in this area. In addition to rigorous follow-up data, we would like to echo the authors’ comment that a homogenous definition of the target population is an essential component of well-designed research. Lack of a clear definition of ductal hemodynamic significance plagues the current body of literature leading, in part, to the confusion that the author s reference.
