Creating a Concierge Urgent Referral Process for Residents in the Post-Acute Setting by means of partnerships with Local Acute Care Systems

Given the growing older population, chronic disease management will continue to be a major focus of the government, health insurers and providers. Historically, the care system consisted of multiple silos with minimal integration, leading to overutilization without significant impact on quality of care. Not surprisingly, older adults have higher frequency of visits to emergency departments compared to their younger counterparts. Most importantly, they experience a higher incidence of poor outcomes due to prolonged ED stays and/or hospitalizations.
