The Dynamic Synergy of Family and Team Member Caregiving for Veterans in Home Based Primary Care

The U.S. Department of Veterans Affairs ’ (VA) Home-Based Primary Care (HBPC) Program provides in-home interdisciplinary, comprehensive, longitudinal primary care for frail, chronically ill, or disabled veterans with complex medical, social, and behavioral conditions. This research describes the role relationships between HBPC team memb ers and HBPC Veterans’ caregivers play in delivering care, from the perspective of HBPC team members.
