23-year-old female with dyspnea, hematuria, and seizure progressing to respiratory failure.

We report a 23-year old female who presented with fever, shortness of breath and abdominal pain. Shortly thereafter the patient developed hematuria, hemoptysis and seizure progressing to respiratory failure. She was found to have aortitis and alveolar hemorrhage. We discuss the clinical manifestations and the diagnostic work up of Takayasu arteritis. The patient's response to therapy and a discussion on treatment modalities of the disease are also included in the report. PMID: 24003540 [PubMed - indexed for MEDLINE]

Authors: Sánchez Ruiz P, Martín Villaescusa C, Duat Rodríguez A, Cantarín Extremera V, Ruiz-Falcó Rojas ML Abstract Primary trochlear headache is a little-known cause of periorbital headache described in adults. It can involve very disabling pain. In addition, it can be associated with other types of headaches, making them even more difficult to identify. To diagnose this pathology, it is necessary that the examination of the trochlea be incorporated into the usual clinical practice of the patient with headache, which will allow the establishment of an adequate treatment. The case...

Abstract A 73-year-old Caucasian man presented with a pink, pearly papule anterior to his right ear. He was a well-educated, retired stockbroker. Biopsy revealed an infiltrating basal cell carcinoma (BCC). The dermatologist repeatedly attempted to contact the patient encouraging treatment. After ignoring calls and letters, he was lost to contact with dermatology for 10 years. In the interim, the patient presented to the emergency room after discovering maggots in his ear. He subsequently consulted a head and neck surgeon but refused the recommended surgical excision. Although still operable, by this time the patie...

We present a case study of a 74-year-old male who presented with right side flank pain and inguinal lymphadenopathy. The patient was diagnosed with low-grade follicular B-cell lymphoma with a low proliferative index (LGFL-LPI) and focal high proliferation index. Approximately 6 months later, the patient was diagnosed with LGFL-HPI with evidence of transformation into DLBCL of activated B-cell immunophenotype within the same lymph node. Separate fluorescence in situ hybridization (FISH) studies performed on the initial biopsy, the low grade component of the subsequent sample, and the transformed component of the subsequen...

Conclusion:18F-FDG PET/CT improved imaging and detection of pulmonary involvement and through TLG value revealed the deleterious smoking effect. The fact that TLG neither detected patients with clinical symptoms and functional impairment nor identified those requiring treatment once again confirms that in pulmonary sarcoidosis the link between activity, severity and decision to treat still eludes us.
