Up to 4% of patients who receive an initial implantable cardioverter-defibrillator (ICD) or receive a generator replacement have an elevated defibrillation threshold (DFT). The current use of DF-4 devices and a trend toward implantation of single-coil vs dual-coil defibrillator leads have negatively affected simple programming changes that can improve DFTs. Mitra (DOI: https://doi.org/10.1016/j.hrcr.2018.10.006) shared a case of a 48-year-old man with arrhythmogenic right ventricular cardiomyopathy and refractory ventricular arrhythmias with an ICD the required battery replacement.
