Resection First

The imaging findings are classic for a vestibular schwannoma: tumor arising from the internal acoustic canal that is hypointense on precontrast and hyperintense on postcontrast T1 magnetic resonance imaging scans.1 The presence of hypointense cysts in the periphery of the tumor is also consistent with a vestibular schwannoma.2 Given the classic imaging findings, tissue confirmation before making a treatment recommendation has little value. Because the local control with either radiation therapy or surgery is excellent,3,4 clinicians should prioritize risk for side effects and preservation of cranial nerve function when deciding between treatments.
