We appreciate Dr Palmer ’s thoughtful response to our manuscript, which found baseline pain variability to be a significant predictor of the abdominal pain placebo response in irritable bowel syndrome (IBS) with constipation.1,2 We agree with Dr Palmer that regression to the mean is part of the placebo effect, but we wo uld argue that response to placebo is much more complex than measurement artifact or spontaneous improvement of symptoms.
