Influence of Risk on Reduction of Readmission and Death by Disease Management Programs in Heart Failure

Heart failure (HF) is the leading cause of hospitalization and rehospitalization for adults aged>65 years.1,2 Despite improvement in outcomes due to development in evidence-based HF medication, post-discharge readmissions among HF patients continue to increase.2 Over the past decade, substantial attention has been focused on early readmission following an index admission of HF, especially since the inclusion of 30-day all-cause readmission or death as a major focus of quality improvement and payment reform.
