Does classification system help with clinical decisions about hip surgery?

(Wiley) Determining which patients can benefit from hip preservation surgery (rather than hip replacement surgery) is challenging, but the American Academy of Orthopaedic Surgeons recently published a hip preservation surgery appropriateness classification system. A new Arthritis Care&Research analysis looked at what impact the system might have on clinical decisions.

Authors: Paul R, Nandi A, Roy D, Ghosh R, Sau TJ, Thakur I, Sarkar R Abstract Rheumatoid arthritis (RA) is a multisystem disease with a variety of manifestations. Vasculitis and myositis are two very rare complications of RA. However, the coexistence of both of these complications in the same patient is extremely rare in medical literature. We here present such a rare association of clinical features in a 36 year old male patient with RA. He had RA for around four years before development of these complications almost simultaneously. The patient was treated with rituximab and oral steroids. The myositis component r...

We present a young male, with long standing fever, weight loss, bone pains, hepatosplenomegaly, cytopenias and severe joint pains. With normal peripheral smear and predominant joints involvement, he was started on corticosteroids. The partial response prompted the physician to continue the steroids. After some time, however, joints and bony pains worsened. After referral to us, he was found to have multiple bony lytic lesions and peripheral smear suggested B cell ALL. So, presentation predominantly with musculoskeletal symptoms, a normal peripheral smear and a partial therapeutic response to steroids as treatment of System...

Brandy-García, Jorge Santos-Juanes, Silvia Suarez, Luis Caminal-MonteroAbstractIgA vasculitis is a small-vessel vasculitis mediated by immune complexes. In clinical terms, it is characterised by palpable purpura in the lower limbs, joint involvement in the form of arthralgia or arthritis, and gastrointestinal and renal involvement (this will mark a poorer prognosis in adults). Infectious processes, mainly in the upper respiratory tract, are frequently found to be triggers. On the other hand,...

We present the case of a beekeeper who, after a bee sting, developed arthritis of the interphalangeal joint of the first finger of his left hand. Although the subacute clinical course and the magnetic resonance imaging findings required the differential diagnosis with an infectious process, the rest of the laboratory tests. Other imaging studies and the course, together with a history of a similar episode a few years earlier on a finger of the other hand after a bee sting, enabled us to diagnosis this condition.ResumenEn los apicultores se ha descrito una artropatía inflamatoria de etiología desconocida pero ...
