Snakes and Ladders: Barriers and Facilitators Experienced by Immigrant Families when Accessing an Autism Spectrum Disorder Diagnosis
AbstractThere presently exists limited information or studies on means to support immigrant families as they attempt to access quality services for children with autism spectrum disorder (ASD). An ASD diagnosis is the gateway to support services and adapted interventions. Yet, studies show that immigrant families typically experience longer delays in obtaining a diagnosis and more challenges in gaining access to services. Twenty-four families who immigrated to Qu ébec (Canada) participated in semi-structured interviews in which they listed the obstacles and facilitators encountered in their service trajectory to obtain a diagnosis for their child. Families’ attitudes toward ASD, as a function of native culture, and their advice to other families in a simi lar situation are were also reported. Obstacles most often mentioned by participants were the waiting period for diagnostic services, feelings of social isolation, and a lack of professionals’ knowledge about ASD. Parental advocacy, the competency and expertise of the professionals who conducted t he evaluation, and parental education and awareness of ASD were the most frequently named facilitators. Finally, the most common recommendations made to other parents were to be persistent and proactive. In light of these reported experiences and perceptions, a series of clinical recommendations are provided to guide ASD evaluation practices in multicultural communities.

In this study, we have used a novel DNAzyme based approach to create a larval model of FXS in zebrafish with specific focus on the early developmental window.MethodsFmr1specific DNAzymes were electroporated into embryos to create theknockdown. We report here the derivation of familial iPSC lines from two controls and three ASD patients carrying NRXN1α+/−, using a non-integrating Sendai viral kit. The genotype and karyotype of the resulting iPSCs were validated by whole genome SNP array. All iPSC lines expressed comparable levels of pluripotency markers and could be differentiated into three germ layers.
ConclusionsPrevention efforts to reduce ED utilization, especially for those with ID and ASD with co-occurring psychiatric conditions, is warranted. Primary care providers and case managers should develop care plans to reduce the likelihood for emergency psychiatric utilization and ensure alternative care pathways. ED clinicians may require additional training to address the needs of this population when they present to the ED in crisis.
ConclusionsThese data contribute to identifying an early trend of autism spectrum disorder, useful also for pediatricians.
We describe QoL data gathered using the Pediatric Quality of Life Inventory (PedsQL) completed online by 364 parents of youth with FXS. Parents consistently reported across all gender and age groups that their children experienced the highest QoL in Physical functioning and the lowest QoL in Cognitive functioning. This report condemns the'horrific reality'of conditions and treatment under which many young people with learning disabilities and autism are detained in mental health hospitals,'inflicting terrible suffering on those detained and causing anguish to their distraught families'. It calls for an overhaul of inspections and changes to the Mental Health Act to protect those detained and for the establishment of a Number 10 unit with Cabinet-level leadership to urgently drive forward reform.
