Frailty Management in Community-Dwelling Older Adults: Initial Results of a Trained Nurses Program

Screening frail older individuals and implementing intervention programs is a strategic measure as it could prevent the progression toward dependency.1 –4 Policy makers and health professionals are exploring different options to implement frailty management in primary care.5,6 Specialized hospital care may introduce a recruitment bias in favor of city residents. Involving primary care physicians limits this risk, but in reality the physicians are overburdened, and this limits home care visits. Thus, there is great concern about the equal accessibility of these programs, especially for rural areas and socially isolated older individuals.
