This study evaluated the association between BRCA mutation status and serum anti-Müllerian hormone (AMH) level in young, reproductive-aged patients with breast cancer. Materials and Methods: Patients ≤ 40 years of age with breast cancer and who had known BRCA status and baseline serum AMH level at Samsung Medical Center, Seoul, Korea, were considered for inclusion. A total of 52 BRCA mutation carriers (27 BRCA1 and 25 BRCA2) and 264 non-carriers were selected for analyses. The serum level of AMH was compared according to presence of a BRCA mutation, and linear and logistic regression analyses were performed to eva...

Cancer treatment — and cancer itself — can threaten fertility. This is a tremendously important survivorship issue for many people. As an oncologist, I’m often asked questions about preserving fertility during cancer treatment. If this issue affects you, here is an overview of key options. When should you talk to your cancer team about fertility? Future children may not be foremost on your mind when you are diagnosed with cancer. We report a case of a 42-year-old woman with a history of bilateral ductal carcinoma of the breast who underwent radical mastectomy with chemotherapy and radiotherapy was anxious to conceive. In vitro fertilization was performed with donor oocyte; the patient became pregnant and delivered a full term baby with no congenital anomalies. As very few have a background for providing guidance and treatment in this setting, this report highlights the possibility of improving the quality of reproductive life of young survivors by various assisted reproductive techniques, the importance of fertility preservation and a multidisciplinary team approach.

CONCLUSION: Despite the majority of young breast cancer patients are concerned about the risk of treatment-induced POI and/or infertility, only a limited number of them required to access the fertility unit to undergo cryopreservation strategies. PMID: 30007268 [PubMed - as supplied by publisher]

ConclusionsLow quality evidence supports the use of GnRHa before and/or during chemotherapy to reduce the risk of POI and increase the probability of spontaneous pregnancy in the short term. Further high quality RCTs with more accurate assessment of ovarian reserve are needed to support any practical recommendation.

Couples who create IVF embryos together should consider hiring lawyers before beginning the fertility treatment process, according to reproductive law experts. While unpleasant, much like a prenup, the process can help avoid a messy, complicated legal battle like the “custody” lawsuit against actress Sofia Vergara by her ex-fiancé, Nick Loeb. Few couples take this extra step before attempting IVF. But Vergara and Loeb’s case underscores the need for conscientious couples to prepare for every potential future scenario, no matter how unlikely, said San Diego-based surrogacy lawyer Stephanie...

Jablonski C Abstract Should all women with BRCA1 or BRCA2 genes mutations be considered at risk of prematurely impaired fertility, and thus should a fertility preservation systematically be proposed? Women carrying mutations of BRCA1 or BRCA2 are at high risk for breast and tubo-ovarian cancer. The treatment of a breast cancer at a young age, unrare in this population, is associated with a risk of infertility, due to the ovarian toxicity of chemotherapy, to the recommended duration of hormonotherapy when indicated, and to the time advised before starting a pregnancy. Furthermore, some data in the literature sugges...
