Bacterial load and pathogenic species on healthcare personnel attire: implications of alcohol hand-rub use, profession, and time of duty

KlawonnSummaryBackgroundSince hand hygiene might prevent the risk of bacterial transmission from healthcare personnel attire (HCPA), the present study investigates the effect of alcohol-based hand rub (ABHR) use on bacterial load and pathogenic species on HCPA.MethodsHCPA from doctors and nurses was investigated for bacterial contamination post duty. Samples from distinct areas of HCPA were obtained and analysed for bacterial load and species. A standardized questionnaire was performed regarding time of duty and profession, and ABHR from each ward was calculated according to a national standard.FindingsBacterial load on HCPA (700 samples from 200 HCPA) was found to be up to four-fold higher when wearing for more than one shift. Moreover, doctors had a lower bacterial load on attire compared to nurses. ConclusionsThe observed effectiveness of N. sativa seed extracts against bacteria was found to be dominantly dependent on concentration of thymoquinone. Its efficiency against S. aureus isolates as well as results of cytotoxicity examination against human dermal fibroblasts indicate on its applicability as an antibacterial agent for topical use and motivates further research in this area.Graphical abstract

A library of 14 nontoxic diterpenoid-like compounds (CC50> 70 µM on HepG2 human liver cells) was screened against Mycobacterium smegmatis, Staphylococcus aureus and Escherichia coli to determine antimicrobial activity. CONCLUSIONSReplacing CP for MRSA-colonized patients with an alcohol?-based nasal antiseptic and CHG bathing protocol significantly reduced isolation costs with no concurrent increase in MRSA transmission. In addition, the alcohol-based nasal antiseptic was preferred by staff when compared to nasal mupirocin.

CONCLUSIONSHospital-wide use of the alcohol-based nasal decolonizing agent to reduce the risk of nasal carriage-associated infections resulted in SSI rate reduction beyond the prior iodine-based protocol and improved nursing-care patient accessibility and cost-savings through reduction in CP use.
