The pathophysiological mechanisms underlying the J-wave syndromes (JWSs), including Brugada syndrome (BrS) and early repolarization syndrome (ERS), remain a matter of debate. At the heart of the controversy is the question of whether the electrocardiographic (ECG) phenotypes and arrhythmogenic substrates are due principally to abnormal depolarization, repolarization, or both.
