Long biliopancreatic and short alimentary limb results in more weight loss in revisional RYGB surgery. Outcomes of the randomized controlled ELEGANCE REDO trial

Laparoscopic Roux-en-Y gastric bypass (RYGB) is a standardized metabolic and bariatric procedure. However, there is not consensus yet on the optimal lengths of the 3 bowel limbs to induce maximum weight loss [1,2]. The RYGB, as revisional surgery, can be technically challenging, but skilled surgeons can perform it with acceptable perioperative morbidity as a 1-stage procedure (band removal and RYGB). Making a longer long biliopancreatic limb (LBP), a fairly easy and secure adjustment of the standard technique, could enhance the results of the revisional procedure.
