Lumbar epidural analgesia for labor in a parturient with a history of surgery for lumbar intradural ependymoma: Literature review and case presentation.

Conclusions: Neuraxial anesthesia can be performed safely in patients who have previously undergone lumbar resections of intradural ependymomas. However, the anesthesiologist should place the epidural needle/catheter at a nonoperative level (e.g. above/below). Furthermore, epidural local anesthetics and opioids, as in this case, placed at the L5-S1 level below an L1-L3 prior surgical scar, may diffuse intradurally, bypassing the obliterated surgical epidural space and/or attendant scar tissue. ConclusionsBased on a moderate level of certainty, orthodontic patients treated with Invisalign appear to feel lower levels of pain than those treated with fixed appliances during the first few days of treatment. Thereafter (up to 3  months), differences were not noted. Malocclusion complexity level among included studies was mild. Pain is one of many considerations and predictability and technical outcome are more important, mainly considering that the difference does not seem to occur after the first months of the orthodont ic treatment.

I tried searching but couldn't find a relevant thread. I was wondering if anyone has experience with a hospital employed job that compensates in "collections minus expenses." How exactly does this work in a hospital setting? The hospital is still getting the facility fee for each clinic visit (hospital clinic)/procedure - so I'm not really clear on what "expenses" are typically attributed to the physician. A 36-year-old lady reported a significant, uncontrollable itch inside her nose, which had been ongoing for a few years.
