Endovascular repair of a large ilioiliac fistula using a reversed iliac limb endograft

We describe the case of a 74-year old patient with a high-flow fistula between the left common iliac artery and the ipsilateral common iliac vein presenting with heart failure. The fistula was probably iatrogenic, caused by prostatic surgery one year earlier. We describe imaging-findings on CT angiography as well as the treatment by two back-table reversed stent-grafts. The satisfactory result demonstrated in our case as well as in the literature suggest that an endovascular treatment for this rare condition should be considered as first-line therapy.
