Treatment with Tumor Necrosis Factor Inhibitors in Axial Spondyloarthritis: Comparison Between Private Rheumatology Practices and Academic Centers in a Large Observational Cohort.
CONCLUSION: With the exception of a lower proportion of patients with nr-axSpA newly treated with anti-TNF agents in private practices in comparison to academic centers, adherence to ASAS treatment recommendations for TNF inhibition was equally high, and similar response rates to TNF blockers were achieved in both clinical settings. Introduction: Leukocyte immunoglobulin-like receptor A3 (LILRA3) belongs to the LILR family with unique feature of a 6.7-kb deletion variation among individuals. Frequencies of the 6.7-kb deletion vary widely across populations, but so far it has not been carefully investigated among Han Chinese subpopulations. Furthermore, we previously identified the non-deleted (functional) LILRA3 as a novel genetic risk for multiple autoimmune diseases. The current study aimed to investigate (i) whether frequencies of the LILRA3 6.7-kb deletion differ within Han Chinese subpopulations and (ii) whether the functional LILRA3 is a novel g...
Researchers know human leukocyte antigen (HLA) molecules affect susceptibility to disease in general, and immunological disease in particular. In the case of ankylosing spondylitis (AS), the risk is primarily associated with HLA-B27, with smaller effects from other HLA alleles. Current thinking is that AS is caused by the presence of a genetically primed host because... [Read More]
CONCLUSION: Our results provided a detailed spectrum of all the reported non-MHC AS susceptibility loci in Han Chinese, which comprehensively exhibited the ethnic heterogeneity of AS susceptibility and highlighted that 2p15, ERAP1 and NPEPPS-TBKBP1 region may play a critical role in AS pathogenesis across diverse populations. ConclusionsAnti-TNF therapy slowed down generalized bone loss, in association with clinical improvements, in both diseases. TNF blockade may enhance bone formation and suppress joint destruction. Anti-TNF therapy may act inversely on DKK-1 and SOST. Independent predictors of BMD were SOST and βCTX in RA, whilst CRP in AS.Key Points• One-year anti-TNF therapy halted generalized bone loss in association with clinical improvement in arthritides.• Anti-TNF therapy may inversely act on DKK-1 and SOST.• Independent predictors of BMD were SOST and βCTX in RA, while CRP in AS.
ConclusionIn conclusion, this study shows that, all else equal, higher SC-TNFi treatment persistence in AS is cost effective from a health care perspective and dominant from a societal perspective. Hence, all else equal, prescribing the SC-TNFi with the highest persistence may be considered a cost-effective strategy.

Fourth year student here – trying to figure out which psychiatry residencies to apply to that have relatively low hours – around 40 hours a week at least for PGY2-4, and the lower the better for PGY1 . I need a program that is relatively low volume because I commonly get flares from Ankylosing Spondylitis (about 1-4x a month for about 1-2 days - more in the winter) that require me to sit down, and not be on my feet all day (otherwise I get pain in my lower spine, knees, hips, and wrists)... Psychiatry Programs With Low Hours? Concerns re: autoimmune fatigue.
The head's center of gravity (COG) plumb line (PL) and C7 PL could be simultaneously positioned over the pelvis in adult spinal deformity (ASD) with normal cervical mobility. However, the position of the head in relation to the global spinal alignment has yet to be investigated in ankylosing spondylitis (AS) patients with thoracolumbar kyphosis.
The objective of our study is hence to determine if gait parameters measured in patients with axSpA could predict the evaluation at 18 months of physical function as assessed by the Bath Ankylosing Spondylitis Functional Index (BASFI). This is a prospective and longitudinal study. Sixty patients with axSpA and 30 healthy age- and sex-matched controls will be included. Patients should be aged 18–65 years at time of their first evaluation, followed at Grenoble Alpes University Hospital for axSpA or ankylosing spondylitis, able to walk 180 m without technical help and with stable treatment for at least 1...
In this study, SUA, CRP, creatinine, MCP-1, IFN- α2, IFN-γ, Il-1β, IL-6, IL-8, IL-10, IL-12, IL-17a, IL-18, IL-23, IL-33, TNF-α, and allantoin levels were measured prior to and after 3 months of TNFis treatment in patients with SARDs. The values obtained in the biochemical assays were then tested for associations with the patients’ demograp hic and disease-related data. A total of 128 patients (rheumatoid arthritis,n = 44; ankylosing spondylitis,n = 45; psoriatic arthritis,n = 23; and adults with juvenile idiopathic arthritis,n = 16) ...
