A multilateral approach to copd comorbidities

Background: The majority of patients with COPD have at least one comorbid condition which further complicates the evolution and the management of the disease. The aim of the study was to evaluate and to analyze the prevalence and clinical significance of COPD comorbidities.Methods: The study was conducted in 435 patients with COPD. Spirometric data were analyzed (FEV1, FVC, FEV1/FVC) and BODE, ADO, BODEx, CODEX and DOSE index.Results: Patients had heart failure in 38.62%, hypertension in 50.11%, coronary artery disease in 23.45%, diabetes mellitus in 10.11%, renal failure in 1.15%, rheumatoid arthritis in 3.22%, depression in 4.83%, cognitive impairment in 4.37%, obesity in 29.89% and cachexia in 3.22%. Only 24.65% of patients did not have comorbidities. One comorbidity was found in 23.73%, two in 24.19%, three and more in 27.42%. Conclusions: A significant increase (P = .02) in Lyme arthritis cases was observed at Oishei Children's Hospital of Buffalo. Lyme arthritis may clinically present similarly to other forms of arthritis, such as oligoarticular juvenile idiopathic arthritis, so health care providers should be aware of distinguishing clinical features, which include rapid onset of swelling and patient age. In this study, N-methylated peptide inhibitors F-N(Me)H-L, V-N(Me)F-R and R-N(Me)V-Y were synthesized against ACE, NEP and APN respectively, using their respective physiological substrates.
