Infero-lateral J wave Syndromes: A Reflection of Abnormal Repolarization, Depolarization, or Both?

The pathophysiologic mechanisms underlying the J wave syndromes (JWS), including the Brugada (BrS) and early repolarization syndromes (ERS), remain a matter of debate. At the heart of the controversy is the question of whether the electrocardiographic (ECG) phenotypes and arrhythmogenic substrates are due principally to abnormal depolarization, repolarization, or both.
