Terrified by suffering, tormented by pain.

Terrified by suffering, tormented by pain. Am J Psychoanal. 2018 Oct 29;: Authors: Eekhoff JK Abstract Ferenczi's understanding of the primitive defenses required to cope with trauma, such as introjection, identification with the aggressor, atomization and reckoning, supports the author's clinical observations of patients who introject a pain mother. Introjecting a pain mother assures that the terrorism of the original external event of a suffering mother keeps the internal suffering going, resulting in being tormented by pain. Such an introjection creates technical difficulties for the analyst. A clinical case is presented to demonstrate. PMID: 30374141 [PubMed - as supplied by publisher]

Abstract BACKGROUND: To date, no studies have been published that have assessed the optimal position of sling immobilization after anatomic total shoulder arthroplasty for glenohumeral osteoarthritis. METHODS: Thirty-six patients undergoing anatomic total shoulder arthroplasty for osteoarthritis were randomized to a neutral rotation sling versus an internal rotation sling. The primary outcomes assessed included the Disabilities of the Arm, Shoulder and Hand score; Western Ontario Osteoarthritis of the Shoulder score; Single Assessment Numeric Evaluation score; visual analog scale (VAS) scores for pain and sat...

CONCLUSION:: (1) Thermotherapy on the vascular access reduced the pain caused by needle cannulation in our patients, without complications related to vascular access. (2) We will consider its clinical application in those painful vascular access cannulations at our hemodialysis unit. (3) Further studies are required to assess other potential beneficial effects added to thermotherapy in vascular access cannulation procedure. PMID: 30442083 [PubMed - as supplied by publisher]

If non-operative treatments fail, then operative ankle stabilization may be required to improve pain and function. Operative options include both anatomic repair and reconstruction techniques. Anatomic repair techniques utilize pre-existing ligament remnants that are either reattached or tighte...

Different mechanisms of sensitization of visceral afferent pathways may contribute to the chronic visceral pain reports and associated brain changes that characterize IBS. They include increased gut permeabili...

A female urethral diverticulum is a focal outpouching of the urethra into the urethrovaginal potential space.1 The reported incidence of urethral diverticula in women is approximately 1% to 6%, with the diagnosis typically occurring between the third and sixth decades of life.2 In symptomatic women, the classic clinical triad of dysuria, dyspareunia, and postvoid dribbling is present (the “three Ds”).1 However, women with urethral diverticula are more commonly asymptomatic or present with an array of nonspecific genitourinary symptoms such as urinary frequency and urgency, chronic or recurrent urinary tract inf...

Conclusions: There is moderate-quality evidence that, compared with usual care, a structured education and neuromuscular exercise program improves physical function, quality of life, and the ability to perform activities of daily living. There is low-quality evidence that, compared with usual care, this type of program improves pain. Low-quality evidence suggests that, compared with patient education, a structured education and neuromuscular exercise program improves pain and physical function.A group-based structured education and neuromuscular exercise program may be cost-effective for the nonsurgical management of knee ...
