Salvage sleeve lower lobectomy was initially planned, however, intraoperatively the interlobar pulmonary artery was difficult to separate from the bronchus. To safely spare the upper lobe, we performed ex-vivo sleeve lower lobectomy, followed by auto-transplantation of the upper lobe. No major postoperative complication was noted and the patient has returned to normal life without recurrence for 9 months.
