21.1 Integrated Treatment of Refractory Depression in Youth: A Review of Medication, Psychological Treatments, and Barriers to Optimal Care

Clinical experience and research have shown that major depression is refractory to treatment; meta-analyses of studies that use a single intervention report moderate effect sizes. Forty percent of patients do not benefit from pharmacological-only treatment, and 35 –55% of patients are unresponsive to CBT-only or interpersonal therapy (IPT)-only treatment. Large controlled studies suggest that combination treatment with CBT plus pharmacotherapy offers superior responses and results.
