AbstractPurpose of ReviewProximal humerus fractures are challenging injuries to treat and will become more common with an aging population. The purpose of this paper is to review the recent literature regarding the role of open reduction and internal fixation for this injury.Recent FindingsComplex proximal humerus fractures present a significant challenge in the elderly. The introduction of locked plating has improved surgical management of these injuries; however, complications including avascular necrosis, primary screw cutout, and secondary screw cutout remain obstacles. Newer technologies aim to combat these issues, with promising early results.SummaryComplex proximal humerus fractures call for experienced surgeons and remain challenging with high complication rates with open reduction and internal fixation. Newer fixation methods aim to reduce these complications.
